With the August 2003 conference coming up on the 15th and 16th, we hope to give everyone a copy of their own yearbook to enjoy. To give you an idea of what it will look like, here is one of the pages from last years book.
We encourage everyone to be a part of the yearbook and share their thoughts and experiences with the other families. If you would like to be included this year, I have included a form to fill out with just basic information about the child or adult with Cystinosis, some bits of information about the family and contact information, if you choose to have it listed in the book.
There is room for a total of 4 small pictures or 2 medium pictures. They can be of your choice. Most of the families have sent one of the child/adult with Cystinosis and the other a family picture. That is entirely up to you.
There are a couple of ways you can get the information to me. If you prefer using a computer, you can send it to me through email. You can type the information right in the body of the email and if you have pictures already in the computer then just send them as an attachment to:
